In search of a broad solution, Ascension Health hatched a plan to address the larger issue by drilling down on one singularly difficult chronic condition.
The Solution: Blazing the Telemedicine Trail
As a test case for innovative new approaches to senior care and more effective, low-cost chronic disease management, Ascension Health joined with Amitech and Best Buy to bring a first-of-its-kind home health monitoring service to market.
The AHConnect Portal combined clinical support with remote monitoring of patient conditions to assist caregivers and healthcare providers in supporting patients’ ability to manage their own care independently. Designed for patients with pre-diabetes and Type 2 diabetes, the service featured “Smart” portable technology devices to measure weight, blood glucose and blood pressure. Via a connection with the patient’s computer, this biometric data was immediately available to designated caregivers, providers and AHConnect care managers for monitoring and analysis.
